If you are a student wishing to apply for Special Consideration based on medical impairment (illness that is not COVID-19, injury, or mental health issue), you can obtain a copy of the Health Declaration Form online.

If your medical impairment is due to COVID-19 (either isolating, testing positive, or having COVID‑19 symptoms) you will need to complete the Declaration to Support Special Consideration Application (COVID‑19) instead—please see the Examinations website for more information:

Supporting Documentation for Special Consideration in Final Examinations

You will need to complete Part A of the Declaration yourself and have Part B completed by a health professional. It is strongly recommended that you see the health professional (preferably Student Health) at an appropriate time, i.e. when the symptoms of your illness or injury are apparent.

  • If you have seen Student Health, they will submit the completed form to the Examinations team who will add it to your application for Special Consideration in eVision on your behalf
  • If you have seen another health professional, you will need to scan and upload the completed form to your application for Special Consideration in eVision
